In the body of the e-mail include the following:

• A photo of yourself
• Your full name: (if for a parent, child or pet, please include your name as well as their information)
• Surgery Type/Description:
• Surgery Date (and time if known):
• Hospital/Surgery Center Name:
• Surgeon/Doctor's name:

Debbie will complete your prep in the turn around time purchased and I (carina) will e-mail you the completed session document with your before & after results.
